Call for Award Nominations

Physician of the Year Award & Nurse Practitioner of the Year Award

OLTCC requests nominations for outstanding Physicians and Nurse Practitioners in Long Term Care. The Long-Term Care Physician of the Year Award and Nurse Practitioner of the Year Award both recognize a member of Ontario Long Term Care Clinicians (OLTCC) who demonstrates exemplary patient care in the long term care setting.  

The nominee commits vision, knowledge and passion to improve quality in their organization. The nominee is a leader and an inter-professional collaborator. 

The deadline to submit a nomination is SEPTEMBER 21, 2026, 11:59 P.M. ET.

Physician of the Year Award Guidelines:

The outstanding individual nominated should be:

• A Physician in good standing with the community and profession.
• A current member of the Ontario Long Term Care Clinicians. (If you are unsure if the nominee is a current member, please contact office@oltcc.ca)
• An experienced Physician in Long Term Care for at least three years.
• A proven team leader.
• An effective educator.
• Cannot currently serve on the OLTCC Board of Directors.

Nurse Practitioner of the Year Award Guidelines:

The outstanding individual nominated should be:

• A Nurse Practitioner in good standing with the community and profession.
• A current member of the Ontario Long Term Care Clinicians. (If you are unsure if the nominee is a current member, please contact office@oltcc.ca)
• An experienced Nurse Practitioner in Long Term Care for at least three years.
• A proven team leader.
• An effective educator.
• Cannot currently serve on the OLTCC Board of Directors.

To Submit:

Click REGISTER in the left-hand column and select the appropriate nomination form.

Within the nomination form, please provide the reasons for your nomination and a brief summary, biography or resume about your candidate. Letters of support may be included. Letters of support may come from colleagues, administration, allied health professional, residents and their families.

The recipients will be selected prior to the OLTCC 2026 Conference. The recipients will be notified of the honour beforehand.

The awards will be presented in-person at the Annual Conference on Sunday, October 25, 2026, at 9:05 a.m., at the Hilton Toronto, 145 Richmond St. W., Toronto, ON.
